Hurricane Evacuation Zone Map
A hurricane evacuation zone is a geographic area that has been identified to protect public safety based on the potential for storm surge. Storm surge is historically the most deadly of all hurricane hazards. If a hurricane evacuation occurs, it will be ordered by Zone (A, B, C,D) as shown on these color-coded maps.

AHCA Assisted Living Facility & Nursing Home CEMP Submittals
• Emergency management plan resubmittal review fee: $250
• New emergency management plan review fee (new licensed facilities): $500
Send plans in Word or PDF format to des.logistics@putnam-fl.gov.
Once submitted and approved for review, an invoice will be sent to the facility.
When payment has been received, the CEMP review will be performed, and an approval or denial letter will be sent. In the event of a denial, highlighted areas will need to be corrected, and the CEMP will need to be resubmitted.
All CEMP reviews have a minimum 90-day lead time.
CEMPs that do not meet formatting requirements will not be invoiced or reviewed.
